Short answer
Chapter 245D is Minnesota's licensing standards framework for specified home and community-based services for people with disabilities and people age 65 or older. It governs much more than a home: it sets duties around rights, service planning, health services, incidents, staff competence, records, and program oversight. A 245D license is distinct from a property license, waiver eligibility, service authorization, and MHCP enrollment.
A licence is one layer of the system
DHS licenses specific basic and intensive support services under chapter 245D. Most are funded through Minnesota Medicaid waiver programs. A county, Tribal Nation, or managed care organization typically acts as the lead agency around assessment, planning, authorization, and case management.[1][3]
Residential language is easy to collapse. In family adult foster care, the adult foster care license holder lives in the home. In a community residential setting, the 245D license holder does not live in the home. A separate 245D provider may also deliver services in a home held by another license holder. The license combination and service model determine which rules apply.[1]
The work follows the person, not the form
The record begins with the person and the services they are authorized to receive. Plans, assessments, health information, contacts, rights, authorizations, incidents, progress, and reviews become a connected paper trail. The form is evidence of work and decisions; completing a form is not itself the service.[4][5][6]
Direct support staff deliver day-to-day support and document what occurred.
The designated coordinator coordinates and evaluates individual service delivery.
The designated manager oversees program quality and compliance.
The case manager and support team remain essential external partners.
Why small providers feel the paperwork first
A small operator may have the same categories of duty as a larger agency but fewer administrative hands. nnipa is a Minnesota-first 245D document-operations system built for that gap. It reads source documents instead of waiting for staff to type them again, prepares pre-filled—not finished—packets, flags missing information, and keeps review dates visible. The result is time returned to judgment and support, with the human still responsible for the record.
Questions people ask
Is 245D the same as an adult foster care home license?
No. They can interact, but chapter 245D licenses specified services; adult foster care and residential-setting licensing has additional 245A and Rule 203 requirements.[1][2]
Is 245D only for residential services?
No. DHS lists both basic and intensive services, including nonresidential services. Confirm the current service list on the DHS licensing page.[1]
