245D admission source-document checklist

A printable working checklist for the documents and facts a Minnesota provider should identify before preparing an admission record.

Four paper tabs connected into an ordered checklist
Short answer

Before filling forms, identify the current support plan and service authorization; assessments; identity and contact authority; case manager and team contacts; health, medication, allergy, and emergency information; risk and abuse-prevention information; rights and financial authorizations; provider policies and service agreements; and the dates that control initiation and review. Not every item comes from one document.

Documents to request and version

Use this as a source-request aid, not a universal legal form list. The person's service, plan, payer, setting, and risk determine what is applicable. Section 245D.095 supplies the record categories; section 245D.071 supplies key planning duties.[1][2]

  • Current support plan, addendum, service agreement, and authorization span.
  • Current assessments, including person-specific risk and self-management material where applicable.
  • Admission identity, legal representative, emergency, case manager, and chosen team contacts.
  • Health conditions, allergies, medications, prescriber orders, protocols, and health-provider contacts.
  • Rights orientation, funds/property authorization, releases, residency or service agreements.
  • Individual abuse-prevention and positive-support information.
  • Provider policies and program-level plans referenced during orientation.

Facts to confirm rather than infer

Confirm the service-initiation date, decision-making authority, emergency-contact role, service location, license-holder relationships, communication needs, medication responsibilities, and meeting dates. If a field is absent, mark it for resolution. 'Not in source' is more useful than a plausible guess.

Turn the checklist into one review queue

nnipa automates this exact boundary. It registers the source documents, reads supported facts once, carries them across the applicable forms, and generates a Need-Info report for the human decisions and missing evidence. The strongest time saving is not faster typing; it is removing repeated searching while keeping every answer traceable.

Questions people ask

Do I need every document before service can start?

Some actions are required before or upon initiation, while others have later timelines. Apply the current statute, plan, service, and setting to the particular person; do not treat this checklist as legal approval to start.[2]

Can I use old provider forms?

Verify that the form reflects current law and your current program. A familiar blank form can still be outdated or incomplete.[3]
